MEDICAL INFORMATION

In the event that Grassy Creek Christian Church / Creek Kids Preschool has the need to seek emergency treatment for your child, the following information is required to be on file, submitted by the parent/guardian. Please submit this form at the time of your Registration.

In the event that medication needs to be administered at school (example: inhaler), you will need to complete this form, print it, and send the form and the medication to school in your child’s backpack. 

All staff are CPR and AED certified. An AED is onsite.
